General Information

We are located inside the Elements Health Club.  We maintain our hours separately from the club, so when you arrive please call if the front door is locked.


Visits are by appointment only.  Scheduling of visits, date changes, and cancellations can all be performed online.  However if you need immediate assistance in scheduling, please contact us at 360-347-6322 during normal business hours.

Please note we require at least 24 hours notice of cancellation or a "no-show" fee may be assessed.

Questions about your visit

I'm in urgent need of a physical, can I just drop in?

In order to accommodate all patients, we require that services be scheduled in advance.  Additionally, we offer mobile services and on-site testing for businesses that have multiple employees in need of testing, therefore we want to make sure we are in the office and ready for you when you arrive.

My employer scheduled me through eScreen or FormFox, will I need to pay?

If your employer scheduled services through eScreen or FormFox, you will not be billed directly by Spectrum Health & Wellness for those services.


I need multiple services performed, can I do it all in one visit?

Yes, just select the appropriate services needed when scheduling online.  This will ensure that adequate time is allowed for your visit with the physician.


I need to cancel my appointment or reschedule, what should I do?

All scheduling services can be conducted online.  If you have additional questions or need support, please contact us by phone during normal business hours.

Health plans & Payment

Payment is due at the time of service, except for those patients who were scheduled by their employer through eScreen or FormFox.

Credit card payments are accepted for Visa, MasterCard, and American Express.  Please contact us for service rates.
